Medical Chart Dividers, Side Tab, 1/6th Cut, Collated Set

Chart dividers are designed for maximum organization within a file folder. Medical file dividers provide quick and easy referencing for access to information. These sets consist of 5 brightly colored tabs/dividers plus 1 blank, writable/erasable tab. All tabs are printed with the same title on both sides and are reinforced with a tear-proof Mylar. Tab Position 1: History/Physical (Medium Blue), Tab Position 2: Progress Notes (Light Yellow), Tab Position 3: Lab/X-Ray (Light Green), Tab Position 4: Hospital (Red), Tab Position 5: Insurance (Clear), Tab Position 6: Blank (Manila). Each divider has a mylar-reinforced strip along the hole punch. Each tab has a mylar coating with print on both sides.
